Starting in 2026, Cisco is partnering with Carrot to provide all family building benefits, including medically-necessary and elective fertility care, preservation, adoption and surrogacy.

Fertility and preservation

Carrot helps you plan for your future and build your family on your own terms, whether at home or with trusted specialists. Services include:

  • Personalized navigation: One-on-one Care Navigator support to help you understand your options, coordinate care, manage logistics and find in-network providers.
  • At-home support: Access to precision ovulation tracking, discounted fertility tests, and sperm testing kits.
  • Fertility preservation: Egg, sperm, and embryo freezing and storage with guidance from trusted specialists and clinics.
  • Expert guidance: Unlimited chats with reproductive endocrinologists, urologists, fertility nurses, nutritionists, LGBTQ+ family-forming specialists, and mental health professionals.

Adoption and surrogacy

Forming your family through adoption or surrogacy can be an incredibly meaningful and rewarding experience, but it can also feel complex. With Carrot, you'll have expert guidance and resources to help you navigate the process with:

  • Personalized navigation: One-on-one Care Navigator support to help you understand your options, manage logistics and find in-network providers.
  • Access to experts: Get help from adoption and surrogacy experts, as well as mental health professionals.
  • Dedicated one-on-one support: Work with your dedicated Care Navigator via messaging, video chat and phone.
  • Exclusive savings: Save on agency fees and waived application fees with select attorneys and agencies.
  • Unlimited learning resources: Get access to curated articles, videos and interactive guides.

What's changing

Medically-necessary fertility care and preservation journeys

  • Coverage through Carrot: Medically-necessary fertility coverage that is currently provided through UnitedHealthcare (UHC) will transition to Carrot on January 1, 2026. Kaiser Permanente members will continue to receive coverage for medically-necessary fertility care through the Kaiser network.
  • Provider network: UHC members will need to use Carrot's provider network in order to be reimbursed; no out-of-network coverage is available.
  • Lifetime maximum: For UHC members, there will be a combined lifetime benefit maximum of $50,000 that applies across all medically-necessary and elective fertility, preservation, adoption and surrogacy services (previously, each benefit had separate maximums).
  • Paying for care: For UHC members, you'll pay your provider directly at the time of service, then submit your claim to Carrot for reimbursement.
  • Cost sharing: Benefits are still subject to your medical plan's deductible, coinsurance and out-of-pocket maximum.
  • Storage benefits: The preservation storage limit is increasing from one to three years for those enrolled in a UHC medical plan.
  • Already receiving medically-necessary fertility care or preservation? If you started a medically-necessary fertility cycle or began using the preservation benefit before January 1, 2026, call Carrot at 888 828 7134 to get support through this transition and determine if your current provider is in network. If your provider is out-of-network, you may be able to apply for continuity of care to complete your cycle with your current provider (subject to the combined lifetime maximum). Any claims you incur through December 31, 2025, will need to be submitted to UHC.

Elective fertility and preservation journeys

  • Coverage through Carrot: All elective fertility and preservation journeys will transition from Optum to Carrot beginning January 1, 2026.
  • Provider network: You will need to use Carrot's provider network in order to be reimbursed; no out-of-network coverage is available.
  • Lifetime maximum:
    • For UHC members, there will be a combined lifetime benefit maximum of $50,000 that applies across all medically-necessary and elective fertility, preservation, adoption and surrogacy services (previously, each benefit had separate maximums).
    • For Kaiser members, there will be a combined lifetime benefit maximum of $50,000 that applies to elective fertility, preservation, adoption and surrogacy services.
    • Any medically-necessary and elective fertility, preservation, adoption and surrogacy services you have previously used through Cisco will count towards your lifetime benefit maximum.
  • Tax changes: Reimbursements will be subject to tax for elective fertility care and preservation.
  • Paying for care: You'll pay your provider directly at the time of service, then submit your claim to Carrot for reimbursement.
  • Already receiving care or using preservation services? If you started an elective fertility cycle or began using the preservation benefit before January 1, 2026, call Carrot at 888 828 7134 to get support through this transition and determine if your current provider is in network. If your provider is out-of-network, you may be able to apply for continuity of care to complete your cycle with your current provider (subject to the combined lifetime maximum). Any claims you incur through December 31, 2025, will need to be submitted to Optum.

Adoption and surrogacy journeys

  • Coverage through Carrot: All adoption and surrogacy journeys will transition from Optum to Carrot beginning January 1, 2026.
  • Provider network: You will need to use Carrot's provider network (e.g., agencies and attorneys) in order to be reimbursed; no out-of-network coverage is available.
  • Lifetime maximum:
    • For UHC members, there will be a combined lifetime benefit maximum of $50,000 that applies across all medically-necessary and elective fertility, preservation, adoption and surrogacy services (previously, each benefit had separate maximums).
    • For Kaiser members, there will be a combined lifetime benefit maximum of $50,000 that applies to elective fertility, preservation, adoption and surrogacy services.
    • Any medically-necessary and elective fertility, preservation, adoption and surrogacy services you have previously used through Cisco will count towards your lifetime benefit maximum.
  • Tax changes: Reimbursements will be subject to tax for elective fertility care and preservation.
  • Paying for care: You'll pay your provider directly at the time of service, then submit your claim to Carrot for reimbursement.
  • Already going through an adoption or surrogacy journey? If you began an adoption or surrogacy journey before January 1, 2026, you may be able to apply for continuity of care and finish your journey with your current provider (subject to the new lifetime maximum). Any claims you incur through December 31, 2025, will need to be submitted to Optum.

What this means for you

If you're enrolled in a UHC plan

Your coverage for medically-necessary and elective fertility and preservation benefits will move from UHC (for medically-necessary services) and Optum (for elective services) to Carrot beginning January 1, 2026. No out-of-network coverage will be available.

If you're enrolled in the Kaiser Permanente HMO (California)

You will continue to receive coverage for medically-necessary fertility care and preservation through the Kaiser network. Elective fertility care and preservation, as well as adoption and surrogacy journeys, will move from Optum to Carrot beginning January 1, 2026. No out-of-network coverage will be available.

If you're enrolled in the UHC Hawaii plan

Your first cycle will be covered through your UHC Hawaii plan and will be subject to your plan’s deductible, coinsurance and out-of-pocket maximums. You can access additional coverage through the Carrot network, subject to the combined $50,000 family building benefit lifetime maximum. No out-of-network coverage will be available. To determine if your current provider is in network and get support, call Carrot at 888 828 7134.
